Insurance Lowry Speech Therapy offers direct billing to insurance companies. We are a provider for Anthem Blue Cross Blue Shield and Medicaid. We will look after pre-authorizations, physician referrals and direct billing once we’ve verified speech therapy benefits under your plan. You will be responsible for any services not covered under your plan as well as co-payments or co-insurance. For insurance billing, we require you to provide all insurance information prior to scheduling your initial appointment. Please complete the Insurance Verification Form and either email or fax it to us so we can verify eligibility/benefits and complete any pre-authorization paperwork needed for coverage. |
